Coexistence of Brugada Sign During a Febrile Episode and an Accessory Pathway: A Curiosity?

Abstract

Fever can unmask or exacerbate an electrocardiographic Brugada pattern. Although its association with an accessory pathway is rare, its identification is important. We present the case of an asymptomatic 26‐year‐old male with a family history of sudden death (father and uncle) who exhibited a Brugada sign unmasked during a febrile episode, with subsequent spontaneous reappearance, in addition to an accessory pathway manifesting orthodromic tachycardia induced during the electrophysiological study. Conventional diagnostic tests were normal. Both the Brugada sign and the left lateral accessory pathway were evident yet intermittent. No malignant ventricular arrhythmia was induced during programmed electrical stimulation. Successful ablation of the accessory pathway was performed. The patient is currently followed on an outpatient basis, receiving quinidine and antipyretics as needed, and remains asymptomatic.
